Preparing for a Visit

At St. Francis Hospital, patients interact directly with experienced physicians and nurses. A full range of additional health care professionals—including nurse extenders, pharmacists, social workers, nutritionists, and pastoral care providers—work as a team to provide personalized care in an environment of warmth, healing, and respect. During your visit, our staff will work hard to make your stay as comfortable as possible, and to care for all of your needs as a patient: medical, emotional, as well as spiritual.
As you prepare for your visit to St. Francis, please keep the following in mind:
- Health Information
Help us provide the besthealth care possible. Be sure to have information on hand, to the best of your knowledge, regarding your present symptoms, past illnesses, hospitalizations, medications and other health-related matters. - Health Care Proxy
It's important for the St. Francis staff to know exactly how you want to be cared for, particularly in critical moments. A Health Care Proxy documents your wishes for treatment in times when you can't express those wishes yourself. so that you can complete it prior to your visit. - Personal Items
Plan on keeping only essential items with you during your stay—a robe, slippers, pajamas, and toiletries. Any other valuables, such as jewelry or credit cards, should be sent home with a friend or relative. - Discharge Procedures
You will want to make arrangements for your trip home before the day of your discharge. If possible, have someone bring your clothing to St. Francis the day before you leave. Before leaving, check your room to make sure you have all your belongings. - Payment Options
St. Francis Hospital will bill up to three insurance companies if presented with insurance information and assignment of benefits at the time of hospital admission. St. Francis Hospital now collects co-payments and deductibles from all patients with commercial insurance at the point of service. You will be notified prior to your date of service of your co-payment and deductible. If you are in need of financial assistance, we will work with you to develop a payment plan that works best. Patient satisfaction is of the utmost importance at St. Francis, and our Patient Relations department offers information, consultation, referrals and advocacy for any concerns that may arise during your visit. If you need to obtain any part of your medical records after your hospital stay, St. Francis Hospital requires that an authorization formbe completed to release the information to you or any other person or organization. To be valid, the form must be signed by the patient or his/her legally authorized representative, dated and returned bymail or in person. Click here to download a copy of this form. Upon completion, please return the signed and dated form to St. Francis Hospital, Health Information Management, 100 Port Washington Blvd., Roslyn, NY 11576. There is no charge to send an abstract of your medical records to a physician or hospital; however, there is a fee if you are requesting medical records for your personal use.
